If you are new patient, we want to make sure, your visit to our office is very easy, comfortable and pleasant. As a new patient, you will have to fill some medical and dental history forms, Consent Forms. You can fill the forms during your dental-office visit or you can print and fill the forms in advance. Once you reach our office, please hand these forms to front-desk.
